Varicocele Surgery

A varicocele is a bag of dilated veins around the testis. Not every one needs surgery — the ones causing pain or affecting sperm do.

When to treat

Many varicoceles are found incidentally and can safely be left alone. Treatment is advised when there are consequences.

The microsurgical approach

Through a small incision at the groin crease, the dilated veins are tied under an operating microscope while the testicular artery and lymphatics are carefully preserved. That preservation is what keeps recurrence and post-operative hydrocele rates low compared with older open or laparoscopic techniques.

Recovery

Day-care surgery in most cases. Desk work resumes in two to three days, gym and heavy lifting after three to four weeks. For fertility, a repeat semen analysis is done at three and six months, since sperm take about seventy days to develop — improvement before that is not expected.

Varicocele Surgery — common questions

Does varicocele always cause infertility?

No. Many men with a varicocele father children without difficulty. It matters when semen parameters are abnormal or the testis is shrinking.

Is varicocele surgery painful?

Discomfort for two to three days, managed with simple painkillers. It is a small incision, not abdominal surgery.

Can a varicocele come back?

Recurrence after microsurgical repair is low — considerably lower than after non-microsurgical techniques.

Will surgery improve my sperm count?

A meaningful proportion of men see improvement in count and motility by three to six months, though results are not guaranteed for everyone.
