Why HoLEP is different
Think of the prostate as an orange. TURP scoops out pulp from the middle; HoLEP separates the whole fruit from its peel. Because the adenoma is removed completely along an anatomical plane, re-growth is rare and the improvement in flow is typically the largest of any endoscopic prostate procedure.
- Size-independent — works on glands well above 100 grams
- Minimal blood loss; transfusion is unusual
- Safe for patients who must stay on blood thinners
- Catheter often removed within 24 hours
- All tissue sent for histopathology
Who should consider HoLEP
Men with a very large prostate who would otherwise be offered open prostatectomy, men on anticoagulants, men who have had retention or a long-term catheter, and men who want the most durable single operation available. It is a technically demanding procedure and depends heavily on the surgeon's experience with the enucleation plane and the morcellator.
Recovery
Most patients go home in one to two days, many with the catheter already out. Temporary urgency and occasional leakage in the first few weeks are common while the bladder relearns normal function after years of straining, and settle with pelvic floor exercises. Strenuous activity is deferred for about four weeks.
HoLEP (Holmium Laser Prostate Enucleation) — common questions
Is HoLEP better than TURP?
For large glands, for men on blood thinners and for durability, yes. For a modest-sized prostate, both give excellent results and TURP may be a shorter operation.
Does HoLEP cause incontinence?
Temporary urgency or occasional leakage for a few weeks is common. Lasting incontinence is uncommon and usually improves with pelvic floor exercises.
Can HoLEP be done on a 150 gram prostate?
Yes. Size is not a limiting factor for HoLEP, which is precisely why it is preferred over TURP for very large glands.
How soon will urine flow improve after HoLEP?
Immediately after the catheter comes out most men notice a markedly stronger stream. Burning and urgency take a few weeks to settle.
