Physiological versus pathological
A non-retractile foreskin is normal in most boys under five and resolves on its own; forcible retraction causes scarring and creates the very problem it is meant to prevent. Phimosis needing treatment is one with a scarred white ring, ballooning during urination, repeated infections, or pain and tearing during intercourse in an adult.
Treatment options
Treatment is matched to severity, and the least invasive option that works is tried first.
- Topical steroid cream with gentle stretching: effective for mild to moderate phimosis in many patients
- Preputioplasty: a small widening incision that preserves the foreskin
- Circumcision: definitive, and necessary in scarred or lichen sclerosus cases
- Treatment of infection or diabetes alongside, since uncontrolled sugar drives recurrent balanitis
Paraphimosis is an emergency
If a retracted foreskin cannot be pulled back over the glans and the glans becomes swollen and painful, go to a hospital immediately rather than waiting until morning. Prolonged constriction restricts blood supply. It is reduced manually in most cases, sometimes with a small releasing incision, and circumcision is usually advised afterwards to prevent recurrence.
Phimosis Treatment — common questions
Can phimosis be cured without surgery?
Often yes in mild and moderate cases, with a course of steroid cream and gentle stretching over several weeks.
At what age should a child be treated for phimosis?
Most boys need no treatment before school age. Treatment is considered if there is ballooning, infection or a scarred ring.
Does phimosis affect sexual function?
It can cause pain, tearing and difficulty during intercourse, and it makes hygiene harder. Both are relieved by treatment.
Is phimosis linked to diabetes?
Recurrent balanitis with phimosis in an adult man is sometimes the first clue to undiagnosed diabetes, so a sugar test is part of the assessment.
